Do you think that this no longer happens? The admin team is a decent size, but even there, this is not their full time jobs and they have limited amount of time/resources; cutting corners may still be a problem.
Time is going to be spent regardless of whatever shortcuts are taken. You either spend your time carefully processing the sanction, or you spend your time fixing the mistake, or you spend your time dealing with the fallout of doing nothing. Same with communicating with the community. You spend your time talking things out and coming to a clear understanding/reasonable conclusion, or you spend your time dealing with angry community members that feel like they are dealing with uncaring or harsh admins. The time will be spent, it's just a matter of how you want to spend it.
