Happened to me once... I think this mostly happens when server crashes and everyone rushes to lurk on the forums...

I figured it wasnt anything special or wrong, just thought it was interesting is all. ;)
iif an admin wants to know how to stop this i can provde some code to stop it happening, contact me on my email attached to this profile
A good idea would be to censor the database username and also the file paths in that error return page.
