That's... not exactly true, unless you are actually flashing the IFF. On my IMG, even at full green, BH NPCs will still occasionally demand pirate pilots from me (which is handled I believe, the same way as unlawfuls pirating you)

It's the flight leader though that will make the piracy attempt. When trying to hail a pirate patrol make sure you select the lead ship.

BHG demand 'lawful' stuff. like the police. and they don't care who is flying friend of neutral dude.

Unlawfuls will only demand stuff from neutral people or little green people.

Also the flight leader only responds no matter who you hail in the group.

I'm pretty speedy at setting in my course corrections to a trade lane or a jump gate, so by the time anyone hails me.

I'm already in cruise, about 5k from a trade lane, now.
By the time they speak, demand, and get annoyed, I'm in the trade lane about 20k away or in the other system.

So I've never really had a problem with them.

Another option if you do get them hostile and need to escape:
Select something you want to go to.
Engine kill
Thruster burn.

The AI just doesn't know how to cope with this and the hostile craft will soon fall behind. Do not reengage your engines though, they will catch up again if you do. Versus players, of course, this is pointless.

well one way to avoid them is to just EK on cruise.

the neutral pirate NPCs follow you as if in formation, so if you cruise the kill engines their cruise will power down but NPCs cannot EK. so you'll be a safe distance away, unless your in a barge, so they can't even turn red.
