Yes, this is really disappointing.

I understand what faction leaders feel when their members do things like that, but... no way to control everyone. I'd ask not to transfer behaviour like this to other faction members or the whole clan. If no actions are taken by leaders that can prevent this in future, though... it might be considered as server rules violation.

Yes. From my point of view, leaders are responsible for the actions of her/his people. A leader must ensure that his troops behave while in the server.
