The scariest game I have ever played is the original Alies Versus Predator throughout the Marine campaign. You are literally crapping yourself throughout the entire game, with eerie silence save for the dripping of water and the steady pulse of your scanner. Occasionally you'll hear a door banging in the distance or a spooky pitter patter of feet in some nearby vent shaft or corridor.

The scariest part of any game I've ever played was as you go through the old Delta Complex in Doom 3: RoE. Damn that 'scenery shift' was.... spooky. 'least it gave me extra lighting though.:laugh:
Homeworld: Cataclysm actually made a space fleet RTS scary, superbly well done game. Having a limited fleet, dark atmosphere, and the threat of your own ships quickly turning against you at a moment, combined with the aesthetics of the soundtrack and environment. Plus, quite a few 'out of the dark' attacks (mission 7 especially).
The Legend of Zelda: Ocarina of the Time spooked me so hard the first time I was attacked by a Master Hand (In the Forest Temple) that, after getting the Bow, I freaking left the temple FOREVER. I only returned when I had like all hearts, upgrades and stuff. Next... oh, laugh if you will, but I have a thing with maggots and vermin... Duke Nukem dude. Those little green blobs that grapple on your character and start to munch your face still give me bumps.
Other than those... Hmm... Clock Tower for the Super Nes is really creepy the first time you play it. (Just don't try the one for the PSX, please, you'll wet yourself, only this time, laughing.) Then we got... Oh right, can't forget the ghost mansion on Vampire: The Masquerade - Bloodlines for the PC! Jesus Christ, that level IS DISTURBING. Trust me, the game may not the best software-wise, but that level is... pure win.
Try playing Aliens vs. Predator, especially part 2 with expansion ... Try using headphones also... Real horror! Too bad third sequel never came out... Really good game. And if you like movies and got real thing into it, it's even scarier.
