In terms of overall scaryness Silent Hill 1.
In terms of shock factor(things that make you jump) Resident Evil 2.
Those are the game at the top of my scary list from when I was a kid playing them the first time.
Have to also add Alien Trilogy if you played it now you'd laugh but playing it as a kid that game was disturbing especially when you got killed and it showed an FMV of the way you died.
The most disturbing moment in a game for me though was in Max Payne 1 when he'd just been spiked with valkrie and started tripping... the main reason it disturbed me though was because I too was tripping and was just like :O
My scariest game is dead space couse I havent YET played any scary games. But dead space is not so scary at all. First 2 hours are scary but then you know when those damn zombies are coming.
I remember mine being a Point-and-click. I don't remember it very well, but it was during that obscure fad of FMV games. I really can't remember what the title was, but I remember that it was about that guy who goes with other people in the woods, and get bit by a wolf... either he gets bit on the throat (Dead) or either it's on the arm, where you can keep on the game.
The dead scene was a tombstone with a wolf howling once in a while... *Shudders*

In other news, much more relevant to the thread, I would like to nominate Bioshock. After you get used to it its just plain fun, but being one of the first games ive ever played with graphics of that level I just though it was "too real" at first. Eventually i calmed down and enjoyed a great game.
Also...Pacman....nuff said.
