Often, you will see people with ranks in their names, especially when they are in a military faction. Now I do know that once in a while, official factions give a lot of powers to (respectable) indies, but you know what kind of people I'm talking about. Can one really proclaim to be a general that came from nowhere, trying to boss around everyone and with no superiors?
Ah yes, i remember one guy trying to roleplay a King of the Corsair Empire, or some such position. He even had a lackey to run around us and advise us on what does 'His Majesty' desire of us to do. No thanks.

Just roleplay him as a madman. A lot of people proclaim they are a messiah of some sort, or Napoleon, or whatever. Send them to the Joker's asylum.
I worked up from a dirty little Rogue bugger to an Ace, well Danny is still dirty but he worked for it. I don't see why others dont have to work up and immediatly call themselves Boss, Admiral, Don or whatever.
