I'm not mentioning any names here, just simply stating what was observed while floating around.
The situation first began with a Freelancer/Mercenary, stating:
"I was hired to kill you."
Showing up really at random, blows up a civilian ship to claim a 'Bounty' or such. A lawful in the area responds by escorting civilian transports nearby. Now that the mercenary is labeled a so say 'Hitman' for hire he is considered unlawful, and a threat if he approaches. Thus after a bit of RP with the mercenary's aggressive behavior it results fighter (hired escort) versus bomber (Considered Unlawful due to Actions) . Now after a little while the mercenary begins to lose the fight. Literally out the blue comes a another ship, a gunboat right as the bomber is losing. He says:
"Oh, Escort I remember you firing on my ship"
Then simply engages in PvP to help his friend win the fight (Despite Never Meeting The Individual) . Where is the line drawn for PvP as results of RP versus false RP as an excuse to help someone else win a fight? In short the escort fighter simply disengaged his RP and just docked to log out. After speaking to him before he took off he asked how can you prove insufficient excuses for RP to PvP? If there is no proof of contract how can a mercenary be any different than a terrorists when all he has to do is use "I was hired" as an excuse to kill basically anything? And how do some ships using poor PvP excuses be properly be resolved?
Screen the chat log, IDs, etc etc. First ship might have been hired legit. Second ship depending on its ID looks like it broke PvP rules. Thing is, just because you felt they broke the rules, doesn't mean they did. Screen the stuff and file a report and let the admins sort it.
First ship: check the bounty boards for any bounty that could conceivably apply. If you don't see any, keep a close eye on the bounty board for the next couple of days, and if nobody ever claims the target as a kill, file a report.
Second ship: Depending on their ID this may or may not be a violation.

First ship is 100% sanctionable if the bounty is not on the bounty boards. There will be a sanction for this on a certain Benitez major soon since it was reported two weeks ago.
Second ship is illegal unless it's a terrorist ID.

That would be a no. There was role play prior to the entrance into PvP. Thus, so long as his ID isn't one of the Freelancer ones requiring some form of payment from another non Freelancer he is good. Navy, police, unlawful that can engage the victim, etc etc.

Actually, no - having previously shot the victim (or not, as described in his scenario) is not a valid excuse for destruction (but is for fine/pirate) and thus is not a valid PvP entrance.
This was purposely done to prevent an Eye-for-an-Eye loop
Only in the same combat scenario. If it happens 30 or even 15 minutes later, no. They can make another demand but cannot just shoot. For example my LABC chases a rogue Scylla to Colorado and loses him. Fifteen minutes later (it was actually about 5) I find him, I can't just pewpewpew again. This is not just a hypothetical scenario, this happened and an admin I was ganged with let me know very quickly.

Fun fact: you don't need to make a demand of a Scylla.
Demands are only required before destroying trade vessels. Combat targets are fair game, as long as you RP beforehand--and "Hey, weren't you the guy who got away two hours ago? Get back here, time to finish what you shouldn't have started" is perfectly acceptable RP.
