Subject is clearly in an unfit state of mind. He seems capable of performing any activities presented to him, and usually quite adeptly. He often falls into a ranting state, where he continues speaking about jump holes.

Subject (S): "They're unsafe! Outright dangerous!"
Dr. Rockwell: "Well, yes, Ageira does make that their official stance on jump holes."
S: "No, no, they're ACTUALLY unsafe. They killed them! They killed them all!"

Clearly the jump holes themselves killed no one. Certainly not everyone. When pressed, the subject goes on to a comprehensive list. Over the past month, I've compounded over a hundred unique names. They range from friends, family, and tormentors to doctors, psychologists, scientists, even acquainted. He does often repeat names, which lends some credibility to his stories.
I have limited records from past psychologists to go on. The condition is from at least 5 years ago, but there are no records prior to that. It is unclear if this is because the condition did not exist, or because the records have been lost.
Subject becomes violent when his fears are brought to him, per the classical treatment. Simulations, which the subject knew in advance were only simulations, of people using jump holes have brought him to knock out any parties choosing to go into the simulated jump hole.
In an attempt to make him realize the error in his ways, he was brought into space in New York. His ship and crew entered the jump hole to the Colorado system. The ship returned safely, but the three-man crew did not. No sign of foul play (or violence) on his part could be found. This event is, as yet, unexplained. He remains convinced that he is the only person capable of regularly surviving jump hole jumps, though he claims he prefers specialized equipment (namely armor and shields) on the ship he uses.
Given current state of the subject, and the inability to explain away past events, it is the view of this psychologist that the subject is untreatable.
The following are two possible causes of the subject's traumatized mentality:
If this mental condition has existed since birth or early childhood, it is possible the subject has simply driven away all those he cares about. Likely one of these occasions, early on, he witnessed the person leaving by way of a jump hole, and has since associated the tragedy with jump holes in every case.
It is possible, though extremely unlikely, that the subject really has had the bad luck he attests to. The unexplained case above is a good example. It is theoretically possible that the subject has lost the people he claims to by way of jump hole annihilation of some sort. Most likely, it is isolated, but extremely tragic circumstances. Perhaps there was a trip he missed out on which contained a large number of schoolmates, where a ship somehow went missing.
The most plausible explanation is some combination of these two. A significantly tragic circumstance involving a jump hole may have led to his fear and his eccentricities, due to which further contact has been cut off. This gives the impression that it is caused by the jump holes.

Two days after these notes were last edited, and just two hours before his (as then) next scheduled appointment with Mr. Livius, Dr. Rockwell's flight from Planet Denver to Planet Manhattan via the Colorado -> New York jump hole suffered a mysterious turn of events. The ship was found, empty of passengers, devoid of signs of struggle. The case remains a cold case.
