Having trouble posting a sanction report

Hi all,

Unfortunately this is probably just inexperience on my part but I can't post a sanction report for a server violation.

I start by clicking: Discovery RP 24/7 Rules
RP 24/7 Rule Violation Reports

After that I click: "Sanction Report" in forum topics however I get a message saying that the link is out of date or broken. Is someone able to help me by telling me if I'm doing something wrong.
