this would be number one in my book.
with the ambience and 12 (yeah just those 12 bastards scared the heck out of me several times - the lights were astrobing when they were close. even were dead - like dead dead, not UNDEAD-ish) zombie patients running around, your character entering the conscience and memory of the building itself. BEST HORROR LEVEL EVER! i need to play that thing again <3
vampire bloodlines haunted mansion level was my number one until i played thief 3. wasn't even close after that.
Metro is ...is.........Like.Just scary man. When you alone , most parts you are ,
The envoiriment you proceed in is incredible detailed , with noises , shadows , Some parts really really made me jump.

You go in caves , destroyed buildings ...Allways dark .As you go playing the game you learn to focus on walls and ceiling and they attack form bellow + Your motion sensor its one of the most scary things .Its there but its not there and its there again makin' wierd noises then its not there and sudentaly something jumps on your face and rwarrasrarara!!

I only played Clive Barkers Undying and its not scary:P

CB's Undying isn't scary, but it has an atmosphere that makes you "cautious" if played well. (Use the amulet, for example)
As for Metro and AvP, I played both. When you're alone for the first time you watch your motion trackers carefully, yes. But as the guns get bigger so does you courage. When I got the shotgun in both games I sort of killed everything I saw, no scaryness anymore.
I don't know if it was because i was a kid, when I played it but the most scariest games for are: Alone in the Dark and Dino Crises 1, both on PS1. I played most of the games mentioned here, but these two I will always remember.
